Cog VM crash

Hello,

With the latest Cog VM I have had non-reproducible crashes when messing
with continuations; a dump is attached.

Continuations are good at flushing out bugs: back when I implemented
my Control delimited continuation library, we flushed out a few bugs
around Context state management.
