The current refactoring shortcuts are limited to renaming entities (CMD-R +  
CMD-C ..) to support further refactorings we should add two-level shortcuts  
for refactorings:

Rename-Method "CMD-R + CMD-M" should become  "CMD-R + CMD-R + CMD-M"

The shortcut would then stand for "Refactor/Rename/Method" etc.
