cannot upload images..

i have a new image that i am trying to upload images to..

for some reason, i keep getting:

The requested object /pier/90/kk960ph0xzfwmlp31li9l3hwmgsbt4/unknown
was not found on this server.

everything in the setup looks okay, and i can add and delete pages at
will.. i just can't upload files..

any ideas?
