The easiest way would be to use the web framework like Aida/Web or
Seaside.  Maybe they sound too big for such a small thing but they are
for sure the easiest and fastest path. Follow for instance the Aida
tutorial http://ww.aidaweb.si/tutorial.html and you'll see by yourself.

In Aida you even have a WebDateInputField calendar component, from where
you can choose date by point and click.
