[vwnc] StoreForMySQL published to public repository

I've published a version of StoreForMySQL, vw76-0.5, which includes some
patches to MySQLEXDI.  I'm interested in feedback and usage reports.  Be
aware the EXDI's documentation is designed for the 5.0.x client
libraries.  Attempts to use it with other client libraries versions will
not work properly.

Just so they exist somewhere, I've packaged-up the parcels along with
some bonus features:

patches to Store so that FK constraints can be loaded
SQL scripts to fix bad foreign key values
SQL script to install referential constraints in the DB
