Cog/image/build*.sh scripts broken due to changes in Squeak-4.5-All-in-One.zip

An extra level of nesting in Squeak-4.5-All-in-One.zip breaks these scripts. I believe the fix may be to change

SQUEAK45APP=Squeak-4.5-All-in-One.app

to

SQUEAK45APP=Squeak-4.5-All-In-One/Squeak-4.5-All-in-One.app

in envars.sh.

We're still finalizing that structure. It looks like the .app
directory is going to be at the top after all.
