I once worked in this project - a video of the application is available
in german:

http://www.bahntv-online.de/btvo/site/index.php?s=5600&ids=139373

At http://www.railml.org/documents/events/slides/2006-09-22_db_vakhtel-xmlkss.pdf
you can find two screenshots on page 5 and 6.

And here are some more Smalltalk applications:

- GEVAS Software in Munich develops a traffic management tool based on VisualWorks
  (http://www.gevas.eu/index.php?id=1&L=1)
- Objectraku's military planning tool "Sextant" is built using VisualAge

- Versicherungskammer Bayern is using VisualAge for its insurance agency information system "VerIA" since 10 years
  http://groups.google.de/group/comp.lang.smalltalk/browse_thread/thread/e2f6178ebeac91a8?fwc=2
- IBM used VisualAge Smalltalk to build the whole VisualAge line (VisualAge for Java, VisualAge for C++, ...)
- Plopp (which is based on Squeak)
  http://www.planet-plopp.de/
- The Dolphin people are now using their own Smalltalk tool to build portfolio analysis software
  http://www.alchemetrics.org/
- JWARS is a military simulation tool written in VisualAge Smalltalk (just google it)
- chip company AMD is using Smalltalk
  http://www.heeg.de/downloads/vortraege/Vortrag_OOP2006/OOP2006_vortrag.pdf
- MetaCase is written in Smalltalk

We use Squeak/Seaside.  We have an 8200 square foot warehouse, and we do a fair amount of volume (with about 20 workers, including some that work from home).

The look and feel of the website was originally created about 6 1/2 years ago by a (then) 12 year old boy, so it looks a little amateurish.  But whenever we survey our customer base with possible website design changes, the customers are pretty solidly against it.  Probably 95% of our customers are women, and they seem to like the design that was done by a 12 year old.

The site was originally done in DreamWeaver, but even then it was hosted under Squeak/Comanche.  Then several years ago I moved the site to Seaside, without changing the design at all.

RUT-K ("Rechnerunterstütztes Trassenmanagement Konstruktion") is a
multi-user client-server system for the timetable management of big
railway systems.
